ADVANCE REGISTRATION (CLOSED)
To qualify for advance registration discounts, your registration form and payment must be
received by 11:59 PM Pacific Daylight Time, Monday, 19 July 2004. Registrations
received between 20 July (12:00 AM Pacific Daylight Time) and 19 August will be charged at the late registration rate.
CONFERENCE REGISTRATION FEES
Conference registration includes a copy of the IEEE Bioinformatics Proceedings, admission to the technical and
poster sessions, keynote and invited talks, opening wine and cheese reception (Aug. 16), breakfasts, lunches
(Aug. 17, 18 & 19), dinners (Aug. 17, 18), daily refreshment breaks, and eligibility to use the Stanford student
housing and discounted rates at Dinah's Garden, the Cardinal, the Sheration Palo Alto, and the Westin Hotels in Palo Alto.
If you wish to bring a guest for a dinner or lunch, the conference reception desk
will issue the appropriate pass on an available space basis. |

MEMBER AND STUDENT DISCOUNTS
To qualify for membership discounted registration rates, your current IEEE or IEEE Computer Society
card must be presented at the registration desk, Stanford University Memorial Auditorium, when you arrive
to pick up your registration materials. Students must show a valid student identification
card at the conference registration desk to get the student discount. Failure to provide the card will
result in a late registration fee. IEEE Life Members may register at the Student rate. If you are
not currently a member, you can enjoy discounted registration rates by applying for
IEEE, or
IEEE Computer Society membership now.
WIRE TRANSFERS
Attendees can register in advance and pay by wire transfer for an additional processing fee of $50
per wire transfer. Wire transfers must be received by the 11:59 PM Pacific Daylight Time, Monday,
19 July 2004 and will not be accepted at the on-site registration. Please contact
bioinformatics@computer.org for bank information.
TRAVEL VISA POLICIES
Authors and attendees of IEEE conferences are responsible for obtaining visas from the host country they are planning to
visit. Travelers should check the host country's policies, and if a visa is required, obtain it from the appropriate
foreign consular representative before proceeding abroad. Applicants for visas should generally apply at
the embassy or consulate with jurisdiction over their place of permanent residence, as it may be more difficult to
qualify for the visa outside their country of permanent residence. Once received, visas should be
reviewed to ensure there are no mistakes. Processing and visa fees vary, and fees may not be refundable.
With security policies tightening around the world, adequate time should be allowed for processing visa applications.
Applicants should initiate the process as early in their travel planning as possible.
The embassy or consulate of the conference's host country should be consulted for specific details. Information
regarding U.S. visa policy is available online at
http://travel.state.gov/.
INVITATION LETTERS
The IEEE policy is that invitation letters can be issued only to speakers, presenters, committee members,
IEEE members or persons known by the organizing committee. In addition registration fees must be paid in
full before the letter is provided. Letters will only be sent by US Postal Mail or fax and only to the
individual registered for the conference. If you require a letter of invitation from CSB2004, please log your
request to BIregistration@computer.org
and include your full name for registration verification.
REGISTRATION CONFIRMATION
Your on-line registration will be confirmed by email as soon as your credit card company approves the charges.
Fax or postal registrations will be confirmed by the IEEE Computational Systems Bioinformatics Conference
within 48 hours of receiving your registration. If you do not receive confirmation, please email us at
bioinformatics@computer.org or call 1-650-851-4588.
PAYMENT
Payment must accompany all registration forms and can be made by credit card (MasterCard, VISA, American Express), company
or personal check, and wire transfers (a fee of $50 per wire transfer must be added to the total
amount). Checks or money orders must be in U.S. funds, drawn on U.S. banks, and payable to 'CSB2004'. You
can mail (1579 Fairway Drive, Los Altos, CA 94024), fax (+1-650-851-8643), or submit your registration form
online. No registrations will be accepted over the phone. Forms received without payment will not
be processed.
ON-SITE REGISTRATION
On-site registrants will pay late registration fees. Forms of payment accepted on site: credit cards
(MasterCard, VISA, American Express), traveler's checks, company and personal checks, and cash. Wire transfers will not
be accepted at on-site registration.
CANCELLATIONS AND SUBSTITUTIONS
Cancellations and substitutions are allowed. However, full registration fees will be charged unless a
cancellation notice is sent by email or fax (+1-650-851-8643) on or before Monday, 2 August, 2004.
A $50 handling fee will be applied to all canceled registrations. Fees cannot be refunded for registrations
canceled after the deadline. For substitutions send email with the name of the replacement to
bioinformatics@computer.org.
IMPORTANT DATES
- 19 July 2004: Advance registration ends 11:59 PM Pacific Daylight Time.
- 2 August 2004: Cancellation deadline. All cancellations MUST be in writing.
- 15 August 2004: All wire transfers and check payments must be received to avoid
on-site registration and payment.
- 19 August 2004: Late registration ends.
